Pharmacological Actions
Neomycin is an aminoglycoside antibacterial agent with an antibacterial spectrum similar to that of kanamycin. It has potent activity against most Gram-negative bacilli, including Escherichia coli, Proteus spp., Salmonella spp., and Pasteurella multocida, and is also active against Staphylococcus aureus. Pseudomonas aeruginosa, Gram-positive bacteria (other than S. aureus), Rickettsia, anaerobes, and fungi are resistant to neomycin.
Drug Interactions
(1) Antibacterial activity is enhanced in an alkaline environment. Concurrent use with alkalinizing agents (e.g., sodium bicarbonate, aminophylline) increases efficacy but also toxicity. At pH values above 8.4, antibacterial activity paradoxically decreases.
(2) Cations such as Ca²⁺, Mg²⁺, Na⁺, NH₄⁺, and K⁺ may inhibit antibacterial activity.
(3) Concurrent use with cephalosporins, dextrans, potent diuretics (e.g., furosemide), or erythromycin may potentiate ototoxicity.
(4) Skeletal muscle relaxants (e.g., succinylcholine chloride) or agents with similar activity may potentiate the neuromuscular blockade caused by neomycin.
Indications
Aminoglycoside antibiotic. Primarily indicated for conjunctivitis, keratitis, and other ocular bacterial infections.
Dosage and Administration
Ophthalmic instillation.
Adverse Reactions
Neomycin has the highest toxicity among the aminoglycosides, however, toxicity reactions are rarely observed with topical administration.
Withdrawal Period
Not required.
